RiseVT, the prevention program operated by OneCare Vermont, is expanding its Amplify Grant program to Island Pond and St. Johnsbury. The grants provide financial support for local projects focused on health and wellness priorities such as walkable communities, nutrition education, physical activity, stress management, breastfeeding support, and tobacco-free environments.
Jen Grant, RiseVT Program Manager at Northern Counties Health Care (NCHC), will lead the initiative. She will work with a team of community members to identify and expand opportunities for residents to get active, eat well, and build local connections. Grant has served as a Community Resource Coordinator at NCHC since 2018, connecting patients with local resources.
“RiseVT has placed program managers at hospitals and health centers across Vermont with the goal of working with local stakeholders across all sectors of the community to invest in local health and wellness initiatives,” said Marissa Parisi, Executive Director of RiseVT.
Grant said the effort will involve collaboration with OneCare Vermont, RiseVT, and the Accountable Health Community at Northeastern Vermont Regional Hospital, known as NEK Prosper. “I look forward to digging deeper into the infrastructure, policy, and programming changes necessary to address the social determinants that impact the health of those in the Northeast Kingdom, and amplifying local efforts that make the healthy choice, the easy choice,” Grant said.
Organizations in St. Johnsbury and Island Pond interested in partnering with RiseVT or applying for an Amplify Grant can visit risevt.org to connect with Grant.
NEK will be following up with Northern Counties Health Care to see which local organizations apply for Amplify Grants and how the funds are distributed.
